Driving Directions from Interstate 680 South

  1. Drive south on 680
  2. Merge onto Mission Blvd/CA-238N via exit 16
  3. Drive on Mission Blvd for 3.6 miles
  4. Turn left onto Mowry Ave for 2.1 miles
  5. Turn right onto Fremont Blvd for 0.7 miles
  6. Turn left onto Mattos Drive
  7. Drive 0.6 miles and Medical Functional Assessments will be on the right hand side

Driving Directions from 680 North

  1. Drive north on 680
  2. Take the Washington Blvd ext towards Irvington District
  3. Merge onto Washington Blvd for 1.1 miles
  4. Turn slight right onto Fremont Blvd for 2.6 miles
  5. Turn left onto Mattos Drive for 0.6 miles
  6. Medical Functional Assessments will be on your right hand side.
